Our Story
Tria Health began in 2009 as a pilot program designed to help members with diabetes – a group of people who often use several different medications to control their condition. When people with diabetes optimize their medications, it reduces overall health care costs for both the patient and their employers. Those involved in the pilot immediately recognized the value of providing this service, and Tria decided to expand its program to include plan members with other chronic and specialty conditions.

The name Tria Health comes from that circle of care we provide. Tria – meaning three – represents the collaboration among our pharmacist, the member and the member’s physician to improve the health of the member.
Since the beginning, Tria Health has provided personalized pharmacy guidance to help companies, ACOs and health plans reduce their medical costs and improve member outcomes.
